We use a printer
friendly format, however you must set the margins on your printer to a
minimum 1/2" on all sides in order to format properly and not miss
any information at the bottom of the printout.
1.) Click on appropriate link to access page.

command to set your margins.
3.) Use your browser 'PRINT' command to
print to your printer.
4.) Use your Browser 'BACK' command to
return here if needed.
